Hospice care provides compassionate care for people whom are simply aging or in the last phases of incurable disease so they may live as fully and comfortably as possible. The hospice philosophy accepts death as the final stage of life: it affirms life but does not try to hasten or postpone death. It concerns itself with providing the individual and the family with support and choices so that everyone can be fully present.
Hospice care is usually given in your home, but it also may be covered in a hospice inpatient facility. Depending on your illness and related conditions, the plan of care can include any or all of these services:

Hospice services provide care for patients in their place of residence. Qualifying for Hospice Care is determined by our medical assessment team. It involves an interdisciplinary approach to meet the physical, psychosocial, spiritual, and emotional needs of the patient and family, as well as the palliation of symptoms.

• The patient is not dying because we are giving medications; medications are given to control discomfort and manage symptoms.
• Discomfort can often rob you of your life energy.
• There can be fear & concern about giving medications.
• People can live longer and better with good symptom management and the right plan of care.
• Less medication is often needed when symptoms are prevented and treated promptly.
• PRN is a Latin abbreviation the hospice team may use meaning “as-needed” when referring to medications.

Studies have shown that when a person enrolls
in hospice care they are more likely to have
increased family satisfaction and better
symptom and pain management. They are also
less likely to undergo tests or be given
medication they really don’t need or want.

While some may think hospice provides 24hr-7 days a week custodial care or
full-time care this is rarely the case. Although hospice
provides a lot of support, most of the day-today care of a person is provided by family,
friends, or caretakers. The hospice care team is however
available by phone 24/7.
